York General Patient Portal Access
York General believes that every patient should have easy, instant access to his or her health information at any time. We are pleased to present this opportunity through the use of a secure internet portal.
Once you have had an admission event (outpatient, inpatient, etc.) and have given York General your e-mail address, you will receive an invitation via e-mail to login.
Please note that you can only review your health data, the portal is not used for two-way communication with York General or your provider.
The York General portal utilizes the same provider as York Medical Clinic (YMC). York General’s patient portal is FollowMyHealth.
Have someone in your life who helps manage your healthcare? York General needs patient consent for others to have access to their medical records. For example, if a daughter wants to look at her adult parent’s information, York General would need written consent from that adult parent. Medical Records Requests
To request your medical records please print off form, (button below) fill it out and sign. Bring it with you to the HIM department or fax 402-362-0452. Please be sure to include yourself and anyone else you give permission to pick up your medical records.
If you know the name of the physician, insurance or workman’s compensation company your signature is not required. In that case simply call the HIM office directly and state your information at phone 402-362-0433
